Changing your Name on AGSM Records


When students initially enrol at AGSM they are required to provide evidence of their full name. Accordingly, students or alumni requesting to change their name on AGSM records, must also provide documentary evidence. This may be in the form of a marriage certificate, deed poll documentation (documentation showing legal change of name), or statutory declaration. In all instances the original or certified copy is required. Faxed or email requests are not acceptable.

All requests for a name change must be made by completing the Change of Name form which should be forwarded to the Award Programs Office with appropriate documentary evidence of the name change. A name cannot be changed until the appropriate documentation has been received.
